Please allow 45 minutes - 1 hour for your initial appointment.

Please download & complete the New Patient Form before the appointment date for your first visit.

Please also bring in any past diagnostic imaging and reports you have if done within the past 5 years.

In order to provide the best possible care, it is important to get to know you by taking a complete history and perform an appropriate physical examination to determine:

  1. 1.Why you have a problem causing pain
  2. 2.Let you know if we will be able to fix the problem
  3. 3.What it will take to get you feeling better as quickly as possible.

If it is determined that further diagnostic imaging is necessary, the Doctor will provide you with a referral to a local facility. Treatment is usually rendered on the 1st visit.  "At home" recommendations are provided as necessary.


Appointment Cancellations
We have a 24 hour cancellation policy. 
Payment is due at the time of the visit for all services rendered.  We accept cash, checks, MasterCard and Visa.
We are 'out of network' for most insurance companies, however, as a courtesy to you, we will electronically bill your insurance company for chiropractic care received and paid in full.  You will be reimbursed by your insurance company directly.
